Rufina Wu, LCSW, Director of Care Coordination

Rufina earned her Bachelor and Master’s degrees in Social Welfare from UC Berkeley and received Postgraduate Training at The Psychotherapy Institute in Berkeley. Rufina Wu joined CPMC in 2014 as Lead Social Worker. She was quickly promoted to Care Coordination Supervisor at our former CPMC California Campus, and shortly after, became the Social Work Manager for all CPMC campuses. In 2020, she transitioned to the Director of Care Coordination. Prior to joining CPMC, she worked at other health organizations including Kaiser and UCSF. She still has a psychotherapy and consulting practice in Berkeley. 

During her 10 years at CPMC, she has expanded community partnerships with post-acute facilities and social service agencies to help facilitate safe transition of care for patients discharging from CPMC. Rufina has developed standard work processes to improve discharge efficiency and enhance multidisciplinary team communication. She and her management team support staff development and clinical supervision to become Licensed Clinical Social Workers (LCSW) and Accredited Case Managers (ACM). 

Rufina is a new mom with a 14-month old born at CPMC Van Ness Campus. Outside of her work, she enjoys traveling with her family and friends and exploring different local cuisines.
