Keith Lagnese

Dr. Keith Lagnese is Chief Medical Officer for Sutter Care at Home. In his role, Dr. Lagnese serves as a dyad partner to the Chief Executive Officer to oversee delivery of Sutter Health’s home-based care—Home Health, Hospice, Home Infusion, Home Medical and Respiratory Equipment, and Sutter SeniorCare PACE—across 27 counties in Northern California.
Dr. Lagnese has more than 25 years of healthcare experience focused on caring for frail and seriously ill patient populations. Prior to joining Sutter Health, he served as a Regional Medical Officer for Landmark Health, a division of Optum Home and Community Health, from 2021 to 2023. Landmark is a national home-based care model focusing on full risk value-based contracting for seriously ill Medicare Advantage populations.
A national leader in hospice and palliative medicine, Dr. Lagnese previously served as Chief Medical Officer at UPMC in Pennsylvania, where he oversaw hospice and home health divisions, from 2014 to 2021. He was also the founder and Chief Executive Officer of Harmony Hospice based in Carnegie, Pa. from 2003 to 2012.
Dr. Lagnese earned his bachelor’s degree from Tufts University; his medical degree from the University of Pittsburgh; and a master’s degree in Medical Management from Carnegie Mellon University. He remains board certified in Internal Medicine, as well as subspecialty boarded in Hospice and Palliative Medicine.
