CQI is an ongoing, data driven approach to make incremental improvements to processes, patient safety, and outcomes. By implementing CQI methods, facilities can reduce preventable harm, improve efficiency and equity, and foster a just culture. Oftentimes, facilities that lack a just culture tend to have employees that shirk away from reporting medication errors in fear of punishment. However, there are many scenarios where the system can be improved. This cannot happen if nobody is reporting preventable medical errors.
