Series Description
The Patient-Centered Home Care Experience Seminar Series is designed to support the healthcare team in improved communication with their patients, address their role as a leader in their practice, give them tools for communicating with and understanding patients from diverse backgrounds, develop skills in safety for both the patient and the healthcare provider, understand patient’s needs related to their diagnosis and support the relationship between the patient and the healthcare team across SCAH. This 10-part series will be delivered in a live zoom format and will have an interactive experience for attendees to ask questions of the presenter.

Learning Objectives
After attending this activity learners should be able to:
1. Summarize common symptoms of dementia and apply evidence-based best practices in the care for a patient with dementia.
2. Describe the grief, death, and dying process and how to support patients and families going through this process.
3. Demonstrate and teach safe management of patient medications in the home health and hospice settings.
4. Utilize mental fitness check-ins as a tool to promote well-being.
5. Manage patients pain through non-pharmacologic, pharmacologic, and therapeutic methods.
6. Identify patients with sepsis and effectively demonstrate timely interventions.
7. Summarize legal documentation requirements and utilize appropriate documentation in patient records.
8. Develop strategies to remain safe in the workplace.
9. Identify and communicate a team focus to promote individual patient safety.
