Series Description:
This Grand Rounds series showcases cutting‑edge clinical insights, emerging practice guidelines, and late‑breaking research across key medical subspecialties. Lectures focus on the prevention of disease, management and treatment of patients, molecular components with a basic understanding of disease process and evidence-based updates on emerging novel therapies. The series is designed to inspire clinical excellence, support evidence‑based decision‑making, and elevate the standard of care across the continuum.
Session Description:
Up to one-third of patients with PTSD and anxiety disorders do not respond to first-line SSRIs and trauma-focused psychotherapy, yet most non-psychiatrist physicians have limited exposure to the interventional options that have emerged over the past five years. This Grand Rounds will present evidence-based update on three rapidly evolving interventional approaches that hospitalists, emergency physicians, surgeons, and primary care clinicians are increasingly likely to encounter at the referral interface: ultrasound-guided stellate ganglion block for PTSD with emerging signals in long COVID and chronic pain; EEG-guided and accelerated transcranial magnetic stimulation protocols (including MeRT, individualized-alpha rTMS, intermittent theta-burst, and SAINT/SNT) for PTSD and traumatic brain injury; and practical patient-selection, safety, and equity-aware referral pathways. The session integrates current peer-reviewed evidence — including the VA/DoD 2023 PTSD guideline, the Stanford SNT trial, the 2025 SGB meta-analysis, and 2025 disparities data to equip attendees with a usable referral framework they can apply in their own settings.

Learning Objectives- After Attending this Session Learners Should be Able to:
- Identify the indications, mechanism of action, evidence base, and safety profile of stellate ganglion block for PTSD, with emerging applications in long COVID and chronic pain, in order to recognize appropriate candidates encountered in non-psychiatric practice settings.
- Differentiate standard repetitive TMS from EEG-guided and accelerated TMS protocols (including individualized-alpha-frequency rTMS, intermittent theta-burst stimulation, and SAINT/SNT) in terms of evidence quality, FDA status, and applicability to PTSD and traumatic brain injury, in order to describe which patients may benefit from referral.
- Apply a structured patient-selection and referral algorithm — including treatment-resistance criteria, safety screening, equity-aware referral pathways, and cross-modality contraindications — to select appropriate interventional psychiatry services (TMS, esketamine, ketamine, SGB, ECT) for PTSD and anxiety patients in their own clinical practice.
