Description:

This grand rounds lecture, the final in a two-part series, will provide pharmacy staff with a comprehensive review of glycemic control principles and their application within the acute care setting. Evidence supports that standardized processes and protocols enhance patient safety by reducing variability, minimizing medication errors, and promoting adherence to evidence-based guidelines; however, a gap exists in that not all pharmacists currently possess sufficient knowledge to consistently apply Sutter Health Pharmacy's Glycemic Control protocol across acute care sites. Bridging this practice gap is essential to ensuring equitable, high-quality patient care and achieving uniform protocol implementation system-wide.

1. Apply the Sutter Health Glycemic Control protocol in hospitalized patients

2. Recognize pharmacist role in glycemic control in medically complex patients
