Session Description
The Patient-Centered Home Care Experience Seminar Series is designed to support the healthcare team in improved communication with their patients, address their role as a leader in their practice, give them tools for communicating with and understanding patients from diverse backgrounds, develop skills in safety for both the patient and the healthcare provider, understand patient’s needs related to their diagnosis and support the relationship between the patient and the healthcare team across SCAH. This 10-part series will be delivered in a live zoom format and will have an interactive experience for attendees to ask questions of the presenter. This session focuses on navigating the processes of grief, death, and dying with a patient-centered focus.

Learning Objectives
After attending this activity learners should be able to:
1. Describe the grief, death, and dying process and how to support patients and families going through this process.
2. Manage patients pain through non-pharmacologic, pharmacologic, and therapeutic methods.
3.. Identify and communicate a team focus to promote individual patient safety.
