review of all wound/ostomy/continence related referrals from field clinicians and assign the home visits. 

Perform home visits for wound, ostomy or continence patients

Joint visit with staff for home visits for teaching purposes

Education of all new staff on Topical Therapy/Products, Compression, Ostomy care, and NPWT

Routine education updates at staff meetings on topics related to wound/ostomy/continence care

Joint MD office visits with patient upon request

Conference with teams for high risk wound, ostomy, or continence patients

Review of wound photos sent in by staff and recommendations for care as related to wound, ostomy or continence

Regular meetings with the SCAH WOCN team

Chart audits related to wound documentation

Supply referrals for all discharging patients in need of wound/ostomy/continence supplies

Approval of all supply requisitions from field staff
