Sara Trott, LCSW
Program Manager, Sutter Care Management 

Sara Trott is the Program Manager for Sutter Health’s Value Based Care Management (VBCM) program, where she has supported strategic initiatives to advance value-based care across the system. With nearly 20 years of healthcare and leadership experience—including roles at Sutter Health, the Department of Veteran Affairs, the Texas Department of State Health Services and UCSF — Sara brings expertise in integrated care models, healthcare payer strategies, and systems improvement.

She has supported three strategic reorganizations within ambulatory case management and spearheaded efforts to achieve NCQA accreditation in 2018, 2022 and 2025. Sara also developed and implemented a comprehensive behavioral health improvement plan and consistently achieves high quality benchmarks in hospital readmissions, ER utilization, and treatment plan compliance.

Her skillset includes strategic visioning, coalition building, talent strategy, change management, and persuasive communication. She is a Certified Social Worker in Healthcare and a Licensed Clinical Social Worker in California and Nevada. Sara holds a Master of Social Work from the University of Texas at Austin and a Bachelor of Science in Urban and Community Structures from UC Berkeley.

She is a graduate of Sutter Health’s Certified Leader II Program (2024) and the Managing for Clinical Excellence Program (2025), and serves on the Sutter Health Systems Readmission Task Force and Utilization Management Committee.
