We are in the midst of an unprecedented change in the American healthcare system.  Data provided by the US Census Bureau shows that the US population aged 65 years and older is expected to nearly double over the next 30 years.  Sutter Health is committed towards becoming an IHI recognized Age Friendly Health System by the end of 2025.  This pledge begins with a targeted approach at improving home health care for the elderly.  With more than 55 million older adults, $500 billion in annual Medicare spending, and nearly 10 million older adults who are Medicare & Medicaid eligible, it is time for Sutter Health to reform our care model to improve continuity, decrease waste, and prevent needless harm.  This regularly occurring educational series will outline what it means to provide Age Friendly Home Health services, the evidence-based fundamentals of high-quality care this construct is built on, and the ways in which Sutter Care at Home providers may achieve this next level of Committed to Care Excellence for our growing aging patient population.  Over the course of this series, content pertaining to the four M's of Age Friendly Health System care will be covered: What Matters, Mentation, Mobility, and Medications.

  1. Appraise the interconnected nature of delirium, dementia, and depression. 

  1. Devise a cohesive care plan that mitigates risk of harm for all 3 D’s individually and collectively. 

  2. Summarize key points for patients and families so that they may distinguish between the 3 Ds and recognize the overlap.
