Description:
The United States is experiencing a significant demographic shift, with an aging and increasingly diverse population. As of 2020, one in six Americans is aged 65 or older, and nearly 11,000 more reach this age each day. With aging comes more complex healthcare needs, yet health systems often struggle to manage this complexity effectively, leading to disproportionate harm for older adults. This course supports that effort by educating future AFHS champions on the 4Ms framework—What Matters, Medication, Mentation, and Mobility—which guides the delivery of comprehensive, person-centered care for older adults. The course also incorporates reflective learning to help participants recognize and address ageism, while promoting age-inclusive best practices that ensure equitable, respectful care across all interactions.

2. Consider the “why” and “why now” of Age-Friendly quality improvement at Sutter Health.
3. Support the readiness of your team, using Sutter created tools based on IHI and CMS identified drivers of effective and sustainable change.
4. Apply reflective learning strategies to recognize personal and systemic ageist biases and incorporate age-inclusive best practices into everyday healthcare delivery.
