Description:
This education session provides physicians and advanced practice providers with practical strategies to ensure clinical documentation accurately reflects patient complexity and medical decision-making. Accurate documentation is critical for safe, high-quality care: when notes are incomplete or ambiguous, important clinical details may be missed, leading to inappropriate coding, delayed or incorrect treatment decisions, and gaps in communication across the care team. These issues can result in fragmented care, increased risk of medical errors, and reduced quality outcomes for patients and families. Participants will review current Evaluation and Management (E/M) coding guidelines, learn how documentation elements support appropriate level selection, and identify common areas where incomplete notes lead to under- or over-coding. Specialty-specific examples will illustrate how clear, concise documentation supports compliance, accurate recognition of clinical effort, and strong continuity of care.
