Description: 

This in-service addresses documented gaps in pharmacist knowledge and practice consistency related to the safe and equitable provision of total parenteral nutrition (TPN) across Sutter Health. Accurate and standardized TPN management is critical to prevent serious complications such as infection, metabolic instability, and medication errors, which can negatively impact patient outcomes and safety. Participants will review evidence-based indications for initiating parenteral nutrition, essential components of TPN formulation, and clinical considerations including fluid and glycemic management, care for special populations, and strategies for navigating product shortages. The session emphasizes the application of standardized pharmacy protocols to reduce practice variability, minimize compounding and medication errors, and improve adherence to best practices. By strengthening pharmacists’ competence in TPN management, this activity supports improved patient safety, continuity of care, and equitable access to high-quality nutrition therapy.

1. Describe the indications for parenteral nutrition and identify when to initiate therapy.
2. Apply knowledge of parenteral nutrition composition—including macronutrients, electrolytes, vitamins, trace elements, and additives—to patient cases.
3. Analyze patient scenarios to determine appropriate fluid and glycemic management, enteric feeding options, cyclic considerations, and strategies for special populations or TPN shortages.
4. Integrate Sutter Health Pharmacy protocol, including EPIC order entry and verification, to develop and implement individualized patient care plans.
