Ella Schwartz RN, graduated from diploma program in Pittsburgh, PA in 2007. Moved to California in 2009. Worked for Hospice of the East Bay from 2010- 2013 as After Hours RN, Hospice Community Liaison and Educator. 
 

2013, began career with Sutter Health as AIM 2.0 transitions of care nurse in the East Bay. 2016, worked as a transitions of care nurse for Bay Area Transitions Team. 2020 started working at ABSMC as Community Case Manager. 2024 transitioned to supervisor case care coordination at ABSMC.

During thirteen years at Sutter Health have been chairperson for Community Integrated Health Partnership, lead on several projects, sponsored charity phone program.
