Description:
The Care Progression Capstone is an interdisciplinary educational program designed to advance best practices in care progression, throughput, and transitions of care. Current gaps include variability in discharge workflows, inconsistent use of Epic tools, and limited alignment between case management, physicians, pharmacy, and post-acute partners. This session provides data-driven strategies to improve LOS, discharge before noon, and readmission reduction while addressing social drivers of health and implicit bias.
