Megan Fischbach, MSN, RN, PHN, is a Post-Acute Case Manager with Sutter Medicare Direct at Sutter Health, where she supports care coordination and quality oversight across a network of more than 90 skilled nursing facilities serving approximately 150,000 Medicare Shared Savings Program members. In her current role, she works closely with hospital and post-acute care partners to improve care transitions, reduce avoidable readmissions, promote appropriate utilization of healthcare resources, and drive quality outcomes across the continuum of care.

With more than a decade of healthcare leadership and case management experience, Megan has held roles spanning acute care, emergency department case management, utilization management, rehabilitation services, and case management supervision. Prior to her current position, she served as an ED Case Manager and Acute Care Case Manager at Sutter Roseville Medical Center and led a multidisciplinary team of case managers and care coordinators as a Case Management Supervisor at River City Medical Group.

Megan earned her Master of Science in Nursing with a focus in Case Management from Samuel Merritt University and holds a Public Health Nursing certification. Her passion for patient advocacy, population health, and care coordination is complemented by global healthcare experience volunteering in HIV/AIDS clinics and community health initiatives in Nairobi, Kenya. She remains dedicated to advancing collaboration between healthcare settings to ensure patients receive the right care, at the right time, in the right place.
