This virtual webinar is designed to support Sutter Health’s system-wide goals around SDOH by:
1.) Fostering a shared understanding of how addressing SDOH aligns with Sutter Health’s mission to deliver integrated, high quality, care
2.) Raising awareness across diverse teams about the importance of SDOH in driving health outcomes
3.) Providing practical guidance to have effective, patient-centered conversations that build trust and improve patient experience
4.) Learn about Findhelp (community programs search and referral platform) to meet patient's social needs.
Bringing together providers, care team members, and support staff in a virtual setting enables broad participation, encourages dialogue, and empowers staff with actionable strategies to ultimately identify and mitigate SDOH for patients.
