This session will facilitate the integration of the Advanced Illness Management (AIM) and Value Based Care Management (VBCM) teams into one cohesive unit with a shared purpose and direction, while establishing standardized processes to support the integrated team in ensuring continuity of care. Participation in this meeting will prepare the care management team to transform the care management program in ways that help reduce the total cost of care, ultimately enabling us to serve more value-based care (VBC) patients. Implementing standardized processes will also help reduce friction and workarounds, improve efficiency, streamline workflows, and minimize duplication across teams.

6. Describe “Age-Friendly” care as defined by the Institute for Healthcare Improvement (IHI) and other external entities.

7. Learn the Pillars of Age Friendly care = the 4M’s

8. Distinguish how Age-Friendly Home Health (AFHH) care both reinforces current Sutter Health standards and provides tools to improve patient health outcomes.
