Description:
Sexual and gender minority patients bring distinct clinical, social, and communication considerations into the ICU, yet these issues remain underrepresented in critical care education. In this SCCM Southern California webinar, speakers will use practical ICU scenarios to explore disparities in care, relevant comorbidities, HIV, gender-affirming therapies, thromboembolic and cardiovascular risk, and transgender-specific considerations in critical care. The discussion will also address chosen family, surrogate decision-making, communication during loss of capacity, and how prior healthcare experiences may shape trust in the ICU. Attendees will leave with practical strategies for providing evidence-informed, respectful, and clinically rigorous critical care to LGBTQ+ patients.

Learning Objectives
Upon Completion of this learning activity, participants should be able to:
1. Describe key health disparities and risk-factor patterns affecting lesbian, gay, bisexual, and transgender patients that may contribute to critical illness, including tobacco and alcohol use, cardiovascular and pulmonary disease, HIV-related disease, obesity, hypertension, and delayed preventive care.
2. Recognize ICU-specific clinical considerations in LGBTQ+ patients, including alcohol withdrawal, respiratory and cardiovascular disease, HIV-related complications, gender-affirming hormone therapy, thromboembolic and hematologic risks, and relevant anatomic or physiologic considerations in transgender patients.
3. Apply an inclusive and patient-centered approach to critical care that incorporates sexual orientation, gender identity, chosen family, surrogate decision-making, and intersectional differences in risk without treating LGBTQ+ patients as a homogeneous population.
