Description:
The Care Progression Capstone is an interdisciplinary educational program designed to advance leading practices in care progression, patient throughput, and transitions of care. Although Sutter Health has established a standardized Care Progression Playbook to guide patient progression activities across the system, variation in adoption and workflow execution continues to exist among sites and service lines. Current opportunities include improving consistency in discharge planning processes, optimizing use of Epic tools, strengthening interdisciplinary collaboration, and aligning practices among case management, physicians, nursing, pharmacy, and post-acute care partners. This educational activity will provide evidence-based, data-driven strategies to improve length of stay (LOS), discharge efficiency, readmission reduction, and equitable transitions of care while addressing social drivers of health (SDOH), implicit bias, and evolving regulatory requirements.
